Skilled/Rehab Facilities

Residents in a skilled nursing facility may also receive physical, occupational, and other rehabilitative therapies following an accident, illness or hospital stay.  Many of our facilities provide in-house rehab therapies, however, we also use other agencies that provide services directly to our patients within the facilities.

Assisted Living/Independent Living

Assisted living facilities offer a housing alternative for older adults who may need help with dressing, bathing, eating, and toileting, but do not require the intensive medical and nursing care provided in nursing homes.

Nursing Home

A nursing home or Intermediate Care Facility (ICF) is a health care facility for individuals who are disabled, elderly, or non-acutely ill, usually providing less intensive care than that offered at a hospital or skilled nursing facility.  However, nearly all residents in a nursing home have the need for some type of medical oversite.  The staff of nursing homes are supervised by registered nurses and physicians.

Medical Director Services

All of our physicians are either Certified Medical Directors, or in the process of obtaining certification, from the American Medical Directors Association. The American Medical Directors Association, the professional association of medical directors, attending physicians, and others practicing in the long term care continuum, is dedicated to excellence in patient care and provides education, advocacy, information, and professional development to promote the delivery of quality long term care medicine. The association has grown to be a recognized expert on clinical practice and physician leadership in long term care.

Palliative Care

Palliative Care focuses on relieving and preventing the suffering of patients with serious illness. It is appropriate for patients in all disease stages.
